I don't know, but maybe the form is personalised and so they will get a form with the correct format. I know I had to fill in mine and it was pre-populated with Name address etc, it could be that date is also pre-populated.

I do not find it odd that anyone from the LGBT community or people of colour feel the Tory party is the one that is closest to their views. Really? The Tories were the party that introduced "clause 28" and were historically much more hostile towards LGBT people, whereas the left were much more like...
