I think if you had it rewarded by CheckoutSmart and afterwards see the offer available in the app (not forgetting to refresh, ensuring that you're looking at up-to-date offers) then it's worthwhile trying again. In usual circumstances an offer would disappear once they've processed your receipt (where the maximum qty has been claimed, usually 1 for a FREE product).
PS. The chocolate mint Yazoo is now down to half price 50p at Tesco, so only £2.50 to spend when buying 5 of them!

Have you checked the claim form? If still there for purchases today, you can go ahead... but I can't check for you since I've already had these items rewarded.
