The story is remarkably light on detail - they health trust have not identified the country of origin of the patient or the nature of the treatment, or even how long the charge has been outstanding.
According to the NHS website, only emergency care is not charged up front; non-emergency treatment must be paid in advance. Given the lack of information, it's impossible to know how such a high charge could arise. It *could* be a simple as the money is unpaid while the patient's health insurer audits the bill.
Mischief making by the publisher of the story.
